Care Management Support Assistant 2

Humana Gold Plus Integrated is seeking Care Management Support Assistant 2s who will contribute to the administration of care management. The Care Management Support Assistant 2 performs varied activities and moderately complex administrative/operational/customer support assignments.

Responsibilities include:

  • Providing non-clinical support to the assessment and evaluation of members' needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitating interaction with resources appropriate for the care and wellbeing of members.
  • Managing inbound and outbound calls with members, their families, and providers.
  • Performing computations, including data entry, in adherence to established metrics.
  • Decisions are typically focused on interpretation of area/department policy and methods for completing assignments.
  • Works within defined parameters to identify work expectations and quality standards, but has some latitude over prioritization/timing, and works under minimal direction.
  • Follows standard policies/practices that allow for some opportunity for interpretation/deviation and/or independent discretion.
  • Other duties as assigned.

Required qualifications:

  • Must reside in or within a commutable distance to Schaumburg or Chicago, Illinois.
  • Two (2) or more years of administrative support experience in the healthcare industry.
  • Basic knowledge of Microsoft Office programs specifically, Word, Excel and Outlook.
  • Familiarity with medical terminology.

Preferred qualifications:

  • Associate degree.
  • Previous experience working with Medicaid, Long Term Care (LTC), or Medicare members.
  • Bilingual or multilingual: English/Spanish, Arabic, Vietnamese, Amharic, Urdu or other - Must be able to speak, read and write in both languages without limitations and assistance.

Additional information:

  • Workstyle: Hybrid office or hybrid home – This role includes remote work from home with in-office work attendance required 2 to 3 days per week.
  • Work location: Schaumburg, Illinois
  • Work schedule: The department operates Monday through Friday from 7:00 AM to 5:00 PM CST. Team members will be scheduled for one of two shifts—7:00 AM to 4:00 PM or 8:00 AM to 5:00 PM CST, with flexibility required for occasional overtime. No weekend or holiday shifts are required.
  • Travel: No additional travel is required beyond scheduled in-office work attendance.
  • Language assessment statement: Any Humana associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Scheduled weekly hours: 40

Pay range: $42,900 - $54,300 per year
