Job Role Summary

The Social Worker is responsible for providing comprehensive social work services which assist providers, patients and their families to find practical ways of overcoming social, emotional, and environmental barriers to optimum functioning and attainment of health. This position acts as educator for clients, families, community and other professionals regarding disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens.

Essential Functions and Responsibilities

Responsible for providing comprehensive social work services which assist providers, patients and their families to find practical ways of overcoming social, emotional, and environmental barriers to optimum functioning and attainment of health.

Provides ongoing assessment, including gathering comprehensive information to use in developing interventions and treatment strategies; included are psychosocial assessments and referrals and OB assessments and education for prenatal patients.

Implements intervention and treatment plans that promote client well-being and ensure a continuum of care. Planning is based on a comprehensive, culturally competent assessment with interdisciplinary input and includes:

  • Basic needs-referrals to community resources for food, clothes, housing/shelter, utilities, etc.
  • Access to medical care - transportation, medication copays, Medicaid, Medicare Parts A, B and D, QMB and Extra Help
  • Home health care, including referrals to CICOA for in-home services, durable medical equipment ordering, and caregiver support and assistance
  • Placement - assisted living, nursing home, group home
  • Abuse/neglect - assist and refer domestic violence victims, APS and CPS reports, referral to resources for prevention of abuse
  • Legal assistance that includes Medical Legal Partnership referrals, legal aid and notary services
  • Education and employment - assistance with children receiving individualized Special Education Programs, serve as a liaison for school system, referral to GED programs, literacy and high school diploma programs, referrals to vocational rehabilitation and employment search programs

Acts as educator for clients, families, community and other professionals regarding disease prevention, impact of illness and disease progression, advocacy for benefits, health maintenance, and adherence to treatment regimens

End of life - Advanced Directives, Physician Orders for Scope of Treatment (POST), hospice referrals and funeral planning/burial assistance

Financial - Budgeting assistance, SSI/SSDI information, etc.

Maintains records or documentation of social work services, which reflect the client and client systems' pertinent information for assessment and treatment; social work involvement and outcomes with and for clients; and in accordance with care goals and legislative and administrative regulations and policies

Job Requirements

Graduation with a Master's Degree from an accredited school of social work; must be licensed or become licensed as an LSW or LCSW when eligible

Graduation with a Master's degree from a related field plus an active LCSW or LSW license may be accepted in lieu of an MSW for those working in Senior Care

Social work experience in a health care setting dealing with pediatric, adolescent, adult and geriatric patients is preferred

Knowledge, Skills & Abilities

Excellent written and verbal communication skills

Ability to take initiative, work independently and be flexible

Knowledge and skills necessary to provide social work services appropriate to the age of patients served. Demonstrates knowledge of the principles of growth and development over the life span. Possesses the ability to assess data and interpret information needed to identify each patient's requirements relative to age-specific needs.

Analytical, psycho-social assessment, and counseling skills

Knowledge of pertinent State/Federal laws and regulations, community systems and resources

Ability to work cooperatively with multi-discipline teams in situations requiring a high degree of flexibility, creativity, and initiative

Ability to apply social work techniques/principles including evaluation, intervention, and counseling

Ability to perform duties independently

Ability to recognize own limitations and seek assistance/guidance from appropriate resources as needed.
