The Assertive Community Treatment Team (ACT) provides intensive wraparound services to adults with serious mental illness. Clients working with this team have commonly experienced recent hospitalization, incarceration, crisis services, housing instability, or a combination of these. This team is primarily community-based. The Clinician performs a variety of individual, family and group therapeutic interventions utilizing evidence-based practices. This position manages a caseload of clients identifying therapeutic, rehabilitative and case management needs, engaging other team members in meeting these needs as indicated.
As a clinician on our ACT team, you'll spend most of your time in the community meeting clients where they are, including in their homes, residential facilities, boarding homes, and other community locations. You will support adults living with severe mental illness, with a focus on individuals who have recently been hospitalized or have complex needs that are best addressed through intensive community-based care.
