The Home Health C N A is responsible and accountable for providing Quality / No Harm, Customer Experience, and Stewardship by performing or assisting with basic patient care functions and providing appropriate age-specific therapy services in accordance with the plan of care established by the Case Manager.

Primary Accountabilities

• Assists the patient with personal care activities following the plan of care established by the Case Manager.
• Performs HHA/CNA skills as taught/directed by the Case Manager.
• Documents services in an appropriate and real time manner utilizing Telephony.
• Notifies Case Manager with changes in patient condition. Documents changes on narrative note and submits with Patient Visit Log for scanning.
• Transports blood/urine specimens to appropriate laboratory facility
• Assist in Oximetry and Telehealth by providing patient education during the installation and removal process of equipment. Provide cleaning maintenance of all equipment with every removal from home.
• Coordinates referrals by other facilities - goes on-site to complete referral, projection of patient needs, and faxes the referral to the agency. Performs liaison activities.

Qualifications

• High School Diploma or G.E.D.
• Use of Portable Pulse Oximetry unit for random Oxygen saturation readings.
• Telehealth set up and removal competency.
• Previous Home Health Aide experience preferred.
• HHA must have completed a 75-hour training course.
• Criminal History Screening to include Level II Fingerprints
• Current Florida Drivers License, automobile insurance and reliable transportation.
• Must hold current AHA BLS Healthcare Provider Completion Card prior to start date and maintain.
• Current State of Florida CNA Certificate.
