We are seeking dedicated and compassionate Registered Nurses to join our Maternal & Child Health (MCH) Community Health Nursing team. In this role, you will provide home visiting nursing services to pregnant individuals, caregivers, infants, and children, focusing on education, assessment, and support to promote healthy outcomes. You will work collaboratively with the State of Maine's Public Health Nursing (PHN) program, ensuring that all care meets state and federal standards, follows evidence-based practices, and aligns with cultural and linguistic competency guidelines.
Key Responsibilities
Direct Care & Home Visiting
- Provide CHN-MCH nursing services through home visits assigned via the CradleMe referral system.
- Conduct comprehensive physical assessments for each client, including vital signs and head-to-toe evaluations.
- Administer and document assessments such as:
- Perinatal mood & anxiety disorder screenings (Edinburgh Postnatal Depression Scale, Columbia Suicide Severity Rating Scale)
- Domestic violence screenings (Safety & Relationship Screening)
- Growth and developmental screenings (Ages and Stages Questionnaires)
- Infant Safe Sleep assessments
- Offer education on prenatal and postpartum health, newborn care, safe sleep, mental health, child development, and injury prevention.
- Develop, implement, and update individualized care plans, ensuring timely discharge planning and referrals to community resources as appropriate.
Compliance & Documentation
- Maintain accurate, confidential clinical records for each client in compliance with state and federal regulations.
- Adhere to immunization and communicable disease clearance requirements.
- Participate in monthly meetings with PHN staff and nursing providers.
- Ensure all client education and marketing materials are approved by the Department prior to use.
Cultural & Linguistic Competency
- Provide services in accordance with Culturally and Linguistically Appropriate Services (CLAS) Standards.
- Ensure educational materials are available in the client's preferred language.
Professional Standards
- Follow CDC Infection Control Guidelines and Core Infection Prevention Practices.
- Conduct services in compliance with all confidentiality, reporting, and insurance requirements outlined by the State of Maine.
