Pre-Enrollment Support Coordinator - 67016176

This is a responsible professional position which provides support coordination to consumers with special needs, resulting from a developmental disability. Employees in this position maintain orderly client central records in the APD iConnect system, independently perform complex and detailed reviews of multiple documents, and complete multiple tasks related to the Pre-Enrollment Workstream often under tight timeframes and with limited supervision.

The work you will do includes: tracking and coordinating the Crisis Waiver Enrollment (CWE) requests, updating the client's record in APD iConnect, working with families and agencies to obtain missing supporting documentation, providing technical assistance to consumers and their legal representatives, identifying immediate local, state and community resources, issuing written notices of crisis denial or approval, facilitating the iBudget waiver enrollment process, completing closure procedures, updating and maintaining assigned clients' central records, creating and updating the short form support plan, consulting with individuals and families to identify resources, advocating on behalf of the assigned Pre-Enrollment clients, documenting contacts and case management activities, conducting disaster planning and quarterly review activities, coordinating and monitoring the delivery of APD funded supports and services, ensuring appropriate delivery of the service, authorizing and preparing IFS invoices, and completing other duties and projects as assigned.

Minimum qualifications include a high school diploma or its equivalent, a valid driver's license or other efficient means of transportation, knowledge and at least two years professional work experience with developmental disabilities or mental illness, case management experience, and customer service experience. Preference will be given to candidates who are fully bilingual (English & Spanish speaker).

Knowledge of using databases, electronic files, and Microsoft Office Programs, at least two years professional work experience with developmental disabilities or mental illness, case management best practices, ability to plan, organize and coordinate work assignments, actively listen to others, communicate effectively verbally and in writing, establish and maintain effective working relationships, solve problems and make decisions, manage a complex case load, represent the programs, policies, and procedures in an assertive and professional manner, travel for work purposes, prioritize and schedule work efficiently, determine work priorities and ensure completion of work assignments, multi-task and work under pressure, demonstrate skills in case management, and excellent customer service.
