Human Services Program Analyst

The Florida Department of Health is seeking a Human Services Program Analyst to perform direct HIV/STI counseling and testing services on inmates submitting and consenting to have such tests performed in accordance with Florida Department of Health HIV/STI testing Guidelines and processes specimen collections for submission to the State Laboratory.

Your specific responsibilities include performing direct court ordered HIV/STI/HCV counseling and testing services when required, acting as the liaison between the Florida Department of Health in St. Lucie County and St. Lucie County jail, Department of Juvenile Justice and other detention facility officials and subcontractors providing medical and other ancillary services within the detention facility for the purpose of coordinating services and ensuring jail linkage programmatic services, needs, and goals are provided at the highest level of quality and accessible to all inmates where applicable.

The position will assist with data-to-care linkage records and assure linkages & referrals to medical, social services & treatment services. Identify newly diagnosed clients, re-engage clients who have fallen out of care, and assist providers in locating non-compliant clients and bring them back into care and treatment. Ensure all newly diagnosed individuals are linked to care within 30 days. Ensures previously diagnosed are re-engaged into care within timelines, coordinate with local regional linkage coordinator (RLC), HIV/AIDS surveillance and STD program staff to ensure data to care (D2C), molecular monitoring project (MMP), molecular HIV surveillance (MHS), and Ryan White re-engagement line list are worked in a timely manner and complete the D2C collection form to appropriate staff.

The incumbent is responsible to identify barriers to care for and assist HIV-positive individuals with necessary referrals and/or practical aspects of adherence such as transportation, obtaining needed medications, receiving Medicaid or private insurance, etc. Performs other duties as assigned including outreach events and community services.

Required knowledge, skills, and abilities include case manager experience, phlebotomy training, self-motivation, flexibility, PowerPoint presentation skills, exceptional interpersonal skills, good organizational and time management skills, ability to work in teams, ability to complete linkage to care (ARTAS) and disease intervention specialist training, experience with delegating tasks, and comfort in talking to individuals and groups.

Qualifications include willingness to become certified in phlebotomy and complete training on disease intervention specialist and linkage to care ARTAS training, willingness to travel out of county, undesirable neighborhoods and/or between counties, willingness to work before and after hours and during weekends, willingness to serve in special needs shelter during hurricanes and/or any other emergency duties involving natural and/or man-made emergency, minimum of 1-year case management experience and/or patient care coordination/navigation experience, valid driver's license, and passing an additional required background screening conducted for and by the St. Lucie County Jail, including fingerprinting and criminal history checks.
