Job Opportunity

Details and Department: Grants Early Steps. Schedule: Full Time Day Shift - Monday to Friday 8 a.m. to 4:30 p.m., with some flexibility based on the needs of the families we serve. Hospital: Ascension Sacred Heart Pensacola. Location: Remote but MUST reside in or near the Western Panhandle for evaluations, events, and meetings (Okaloosa, Walton, Santa Rosa, or Escambia Counties).

Responsibilities: Coordinate access to health-related services by assisting clients with utilizing available resources. Identify potential clients, complete initial needs assessment, and maintain follow-up to ensure appropriate guidance to health services. Provide informal counseling, education, and social support to clients. Assist clients with utilizing available resources, including scheduling medical appointments, accompanying clients to appointments, and assisting with completion of medical program applications. Develop and maintain strong working relationships with external (referral sources and community agencies) and internal contacts (departmental team members). Maintain accurate client records by documenting all contacts, services provided, and outcomes in order to track all clients and services for future reference.

Requirements: Education: High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences: Ideal will have the following: Bachelor's degree or higher from an accredited university with an emphasis in the areas of psychology, social work, health education, interdisciplinary sociology, early childhood, child development or special education, or an out-of-field degree with a minimum of three years documented experience with young children and families in a teaching, case management or counseling role, or with individuals with special needs and/or developmental delays of any age in a case management role, or a registered nurse without a bachelor's degree, but with a minimum of three years' experience in the same areas required for those with an out-of field degree. This position will require a level II Background Screening for Florida Medicaid.
