This position is an Advanced Practice Registered Nurse who will provide health care services to the unsheltered homeless where they reside as part of a street medicine initiative of the Health Care for the Homeless Program. The duties of the position include providing comprehensive health care to homeless clients via outreach, in clinic setting, and/or via telehealth. The position also requires evaluating and performing complete physical examinations, analyzing and interpreting patient histories, symptoms, physical findings, and/or diagnostic information to develop appropriate diagnoses, diagnosing and treating acute health care problems such as illnesses, infections, or injuries, providing wound care, ordering, conducting, interpreting, and/or evaluating diagnostic tests to identify and assess patient's condition, ordering, reviewing and interpreting laboratory tests to identify and assess patient's condition, prescribing and administering medications based on patient characteristics, efficacy, safety, and cost as legally authorized, making medical diagnosis and prescribing treatment within scope of service, developing treatment plans based on scientific rationale, standards of care, and professional practice guidelines, making appropriate follow ups and referrals, educating patients about self-management of acute and chronic illnesses, tailoring instructions to patients' individual circumstances, providing consultation and technical assistance in evaluating the appropriateness of client care as it relates to primary care, documenting all services in EHR timely and accurately, communicating with medical team and other colleagues, complying with all UDS measures, clinical standards and selected goals for quality of care, providing excellent customer service, completing case management, including record review and lab review, keeping supervisor and Medical Director informed, completing timesheet, EARS, and mandatory training in a timely and accurate manner, completing training to promote professional growth, participating in peer review and provider, team, and other meetings as assigned, and performing other related duties as assigned including emergency duty.
Required knowledge, skills, and abilities include knowledge of nursing principals, practices and techniques, knowledge of human anatomy and physiology, ability to administer prescribed medications, ability to perform complete physical assessments of patients, ability to identify and evaluate illnesses, ability to initiate diagnostic laboratory testing and evaluate the results, ability to manage the care of patients, ability to compile and evaluate medical histories and other clinical and laboratory data, ability to plan, organize and coordinate work assignments, ability to use independent judgment and decision-making, ability to work independently and manage one's own time, ability to use critical thinking, knowledge of customer service principles, ability to communicate effectively both verbally and in writing, ability to develop and maintain effective working relationships, and skill in computer software programs used to facilitate job duties.
Qualifications include a clear and active license in the state of Florida to practice as an Advanced Registered Nurse Practitioner, certification in the State of Florida as an Advanced Registered Nurse Practitioner in accordance with Chapter 464, Florida Statutes, a valid Florida driver's license, access to reliable personal transportation, a Masters Degree, BLS certification, DEA license, three or more years of experience utilizing an electronic health record in a clinic setting, three or more years of experience working as an ARNP in an internal medicine or family practice clinic setting, experience working as a nurse in an outreach setting, experience working at a Federally Qualified Health Center, experience providing clinical care to patients experiencing homelessness, and experience providing clinical care to clients via Telehealth.
