Responsibilities

Direct case management services, workflow, and resources.

Formulate and implement operational strategies and initiatives in the case management program.

Direct and evaluate all services provided, including care plan development, service level determination, and complaint management to achieve performance and quality control objectives.

Provide specialized expertise, ensure resource alignment with system-wide initiatives, and updates appropriate parties on case management strategies, initiatives, outcomes and issues.

Partner with physicians and others to ensure delivery of evidence-based care and reduction of avoidable clinical variation in care. Collaborate with a larger multidisciplinary team to improve quality outcomes using data and documented best practice.

Responsible for resolving escalated issues arising from operations in the case management department.

Requirements

Licensure / Certification / Registration:

• Registered Nurse credentialed from the Florida Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Bachelor's degree in nursing or diploma from an accredited school/college of nursing required.
• Master's degree or combination of Bachelor's degree and experience preferred.
• OR 5 years of experience required.
• 10 years of experience preferred.

2 years of leadership or management experience required.

5 years of leadership or management experience preferred.
