Veteran Directed Care Counselor needed - Syracuse or Oswego, NY

ARISE/Exceptional Family Resources is looking for a caring and compassionate Veteran Directed Care Counselor, to be an advocate and coach for veterans. The Veteran Directed Care Counselor (VDC) serves veterans of all ages and all types of disabilities that are enrolled in the VA Healthcare system that needs nursing home care. The VDC will assist veterans with the opportunity to receive home and community-based services, supporting them to live independently in their homes. While the Veteran manages their own monthly budget to directly hire workers and/or purchase goods and services to meet their needs in their home and community. In partnership with the Veterans Affair Medical Center, The Person-Centered Counselors supports the Veteran's care and service planning.

The ideal candidate will need to have excellent verbal and written communication skills, demonstrate computer proficiency; with the ability to learn new software and some familiarity with home health care services and regulations. Location: Office position- 635 James Street Syracuse or 293 State Route 104E Oswego Ability to work from home/remote: Hybrid schedule pending training after 90 days Benefits: Benefits available Schedule: Monday- Friday (flexible start time from 7:30am-9:00am) Full time: 40hrs weekly Benefits: Benefits available the following month after day of hire

Qualifications: Must be at least 18 years old. Reliable with effective communication and organizational skills Familiarity with home health care services and regulations No experience required but experience working with people with disabilities preferred. Clean driver's license and registered, inspected, insured and reliable transportation. Demonstrated computer proficiency; ability to learn new software Bachelor's degree in human services or related field, or an equivalent combination of experience and education Background check, fingerprinting and DMV record check will be conducted prior to hire by the non-profit agency.
