Summary: This position provides casework services, including assessments, problem identification, and develops a service plan to assist client and family in dealing with medical conditions, following medical recommendations, and achieving maximum social and health adjustment. Duties include: developing and administering needs assessments for clients, documenting in the EMR, working with clients, community partners, and state agencies, including the Division of Social Services and Division of Substance Abuse and Mental Health to reduce barriers in care and addressing social determinants of health.
