Summary: Within this role as Health/Human Service Case Manager II, you will work for a specialized team within the Division of Social Services called the Change Report Center, which includes a call center where clients report changes in their circumstances. You will process these changes and determine ongoing eligibility for all needs-based benefits which includes Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children's Program (CHIP), Subsidized Childcare, State Funded General Assistance (GA), and Temporary Assistance to Needy Families (TANF). In addition, you will counsel, case manage and make referrals for benefits DSS does not provide (such as Child Support, foreclosure assistance, or food closets). Case Managers will fulfill the diverse needs of vulnerable and low-income residents by sharing information about benefits in a timely, accurate manner.
