Summary: Join DSAAPD as a Health/Human Service Case Manager II where you will assist older Delawareans, persons with physical disabilities, and caregivers to get connected with community services to maintain independence in their own homes. Through home visits and evidence-based assessments, you will provide counseling, person-centered care planning, and assistance with accessing social service resources throughout the Aging and Disability Networks Statewide.

You'll be responsible for case management at the full performance level, counseling and assisting clients to meet their personal, social, health, and economic needs. Through applying a person-centered planning approach, you'll make home and/or field visits to interview clients, their families, and/or professionals to create a plan of care that supports the least restrictive setting. Your duties will also involve making referrals, assisting in the application process for a variety of programs, and determine initial and/or ongoing eligibility for services under the Community Services Program.
