Summary: Within this role as a Health/Human Service Case Manager I, you will determine initial and ongoing eligibility for all needs-based benefits which includes: Food Supplement Benefits (SNAP), Medical Assistance, Delaware Healthy Children's Program (CHIP), Subsidized Childcare, and State Funded General Assistance (GA). You'll also work closely with SNAP vendors. In addition, you will provide entry-level counseling, case management, and in making referrals for benefits DSS does not provide (such as Child Support, foreclosure assistance, or food closets). Case Managers will fulfill the diverse needs of vulnerable and low-income residents by sharing information about benefits in timely, accurate manner.
