Summary:

This position supports staff and others by providing helpful information, answering questions, and responding in a helpful and timely manner. Serves as the front line of support for clients and families and helps to ensure they are satisfied. Visit individuals in homes or community to gather and provide information. Advise clients regarding community resources. Consult with supervisor concerning programs for individual families. Transport and accompany clients to shopping areas or to appointments, using automobile. Work side by side with staff and other essential members of the family engagement team to support families to be fully engaged in all aspects of their treatment. May assist clients in identifying and obtaining available benefits and social and community services.
