This C/S Adolescent Treatment Services position sits within the Child and Family Care Coordination Unit and will be a member of the unit's team. Care Coordinators support children and families with care coordination, and link families to resources and services that are clinically necessary in a least restrictive environment to promote positive emotional and behavioral adjustment. This position works closely with the family's formal (e.g., child welfare, juvenile justice, schools, etc.) and informal supports (e.g., neighbors, family members, clergy, etc.) to improve the academic, social, emotional, and behavioral outcomes for their child(ren). This position is typically for 29.5 hours a week; however, staff may request additional hours with HR.
