The Chronic Disease Nurse Navigator is the primary hospital-based clinician coordinating care for patients with conditions like AMI/CABG, Heart Failure, COPD, and others at Midtown, Gateway, and Henderson campuses. This role works with a multidisciplinary team to ensure efficient, high-quality care for these patients throughout their treatment journey. The Nurse Navigator will contact patients, collaborate with providers, and work with the team to remove barriers, improve outcomes, and enhance satisfaction. Acting as a clinician, advocate, educator, and inpatient facilitator, they oversee patient care and serve as a resource for chronic disease management. The role requires knowledge of patient development and care appropriate to the patient's age. They will use evidence-based practices and understand care pathways for these patients.
