Director, Clinical Services (Home Dialysis)

The Director, Clinical Services (Home Dialysis) is a Group-level RN leader for Home and Peritoneal Dialysis, and is responsible for the creation and implementation of programs to improve Home patient quality outcomes with oversight of training, education, project management and support. This position serves as the subject matter expert and escalation point of contact by partnering with the Home & Ops field teammates, to increase focus on patient safety and improve Home patient hospitalization and retention metrics. This teammate has accountability for HHD / PD metrics and improves outcomes by addressing key drivers and coaching local teammates on action plan development and implementation.

Key Responsibilities:

• Identify and surface key drivers impacting patient time on therapy, help problem solve and remove barriers
• Track HHD/PD hospitalization and survey data; share with teams to identify trends, anticipate future needs and draw resources
• Actively cultivate network of HHD/PD divisions leads, program managers and champion
