Who We Need
As a member of the GMH/SUD Integrated Care Team, participates in the development of the integrated comprehensive assessment and Integrated Service Plan (ISP) and implements services to members in accordance with their plan and reviews progress to ensure services are beneficial. Coordinates with involved healthcare, behavioral health and wellness providers and maintains current documentation on members' progress in managing medical and psychiatric conditions.
Job Summary:
- Coordinates with Lead GMH/SUD Coordinator on GMH/SUD program development and needs
- Works directly with assigned Psychiatric Services and Medical Assistance staff to engage, educate, communicate, and coordinate care with service participants, their family, behavioral health, medical and dental providers, community resources and others in ensuring that all services in the Integrated Service Plan are implemented. Develops and maintains and manages the Integrated Service Plan (ISP) within required timeframes.
- Maintains the person's comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).
- Reviews the ICA with the Psychiatric Services and Medical Assistance staff and works collaboratively with other members of the integrated care team when additional services are needed.
- Develops and adjusts the Integrated Service Plan (ISP) based on identified needs and the preferences of the individual for their specified health, wellness and behavioral health goals.
- Arranges for transportation to medically-necessary covered services and registered AHCCCS provider sites.
- Provides assistance and resources to support member access to covered behavioral health and healthcare services and community resources.
- Coordinates and communicates with medical and psychiatric practitioners for individuals in inpatient levels of care or experiencing complex medical and/or psychiatric conditions.
- Participates in regular GMHSU integrated care team meetings and documents actions to modify service delivery based on progress, social determinants of health, health literacy, current gaps in care, and level of self-management of chronic conditions.
- Collaborates with Copa Health Care Management to identify and close gaps in care
- Coordinates with evaluation agencies for members referred for SMI determination
- Provides outreach following crisis episodes and missed appointments for members on the GMHSU High Risk List.
- Assistance in maintaining and monitoring person's court ordered treatment, i.e. COT appointment, court appointments, status reports, and tracking of IP hospital days, etc.
- Collaborates with Copa Health Care Management in the risk analysis process and implementing strategies to reduce utilization of emergency departments and inpatient facilities.
- Serves as the primary point of contact for the caseload.
- Complete required reports and spreadsheets, including COT tracking.
- Maintains the person's comprehensive clinical record, including documentation of activities performed as part of the service delivery process (e.g., assessments, provision of services, coordination of care, discharge planning).
- Maintains documentation consistent with Copa Health productivity levels.
Minimum Qualifications:
- A qualified behavioral health technician, and/or at minimum, a high school diploma or high school equivalency diploma and four years behavioral or healthcare work experience required.
- Bachelor's degree in behavioral health related field and one year of behavioral or healthcare experience preferred. Must have a fingerprint clearance card issued by the AZ Department of Public Safety or eligible to receive one within eight (8) weeks of hire
- Must have reliable transportation, valid AZ Driver's License with state minimum insurance coverage and ability to meet and maintain Copa Health's acceptable driving requirements.
- TB Test clearance required at the time of hire and annually.
- Must complete training and demonstrate competence in Integrated Care skills.
