Who We Need
Immediately looking for Direct Support Professionals to support members with acquiring the skills needed that pertain to community engagement opportunities such as: community outings, volunteering, and pre-employment activities, etc. Provide for social, emotional, and/or physical needs of the member and assist with routine ambulation activities.
Minimum Qualifications:
- Minimum age of 18 years old.
- Requires any combination of training, education, and/or experience equal to a high school diploma or GED.
- A minimum of three (3) months of care-giving experience in the field of disabilities preferred.
- Must pass a pre-employment drug screen, physical clearance and background check.
- Must have, or have the ability to pass the background checks to obtain, a Level One (1) Fingerprint Clearance Card
- Ability to meet Copa Health's driver eligibility requirements, as applicable.
- Must have a current AZ driver's license and a clean 39-month driving record.
- Must be able to lift minimum 50 pounds, stoop and bend comfortably in all positions.
- Must be comfortable assisting in bathing, dressing and overall hygiene of clients.
Job Summary (Including, but not limited to):
- Assist with special appliances and/or prosthetic devices.
- Provide support to ensure members understand their rights.
- Provide opportunities for members to engage in socialization and learning activities.
- Assist members to develop money skills to manage their personal funds, as applicable.
- Transferring individuals from wheelchair to changing table, bed, chairs, etc.
- Provide assistance to members in and out of company vans.
- Transport members during scheduled activities and outings, as needed.
- Facility cleaning includes but not limited to sweeping, mopping, vacuuming, dusting, and laundry.
- Assist with personal hygiene, which may include, changing adult incontinent briefs.
- Maintain body equilibrium to prevent falling when walking, pushing, pulling, reaching, standing, or crouching on various floor and ground surfaces.
- Assess individual behavior and respond appropriately.
- Completes all clinical/program documentation in a timely manner in accordance with policies, procedures, and clinical protocols.
- Adherence to all agency policies, licensure, and training requirements.
- Proficiency in typing, using computer software, i.e., Internet, Therapy tablets, GPS devices, EHR/EMR systems and MS Office.
- To ensure compliance & adequate services, additional job duties may be required to meet the needs of the program and/or department.
