Transition Service Supervisor

As a Transition Services Supervisor, you'll support individuals with disabilities as they take exciting steps into new life phases-like moving from high school to adulthood, from family homes to more independent living, or into new vocational programs. In this role, you'll help with planning, provide guidance, and make sure each person has the support they need to reach their goals.

Bachelor's degree in human services or related field (preferred, but not required)

At least 2 years of experience in areas like transition coordination, supported living, case management, special education, or program development

Understanding of local IDD resources and transition processes

Strong organizational skills to balance multiple cases and timelines

Clear, compassionate communication-both in writing and in conversations with families, schools, and agencies

Valid Colorado driver's license and reliable transportation

It's a bonus if you already know Montrose's service providers, schools, housing resources, or healthcare partners-but if not, we'll help you get familiar.

Assessment & Planning

  • Meet with individuals and families to understand needs in areas like school, work, and daily living
  • Work together to create person-centered plans that highlight strengths and goals

Coordination & Support

  • Help people smoothly move into adult services, supported living, jobs, or day programs
  • Teach everyday skills like time management, self-care, budgeting, and social skills
  • Set up visits to explore housing, workplaces, transportation, and community programs
  • Partner with case managers to make sure people qualify for the services they need

Communication & Advocacy

  • Be the go-to contact for families, schools, and providers
  • Attend meetings (like IEPs) and share updates on progress
  • Speak up for access to education, employment, and community opportunities

Tracking & Reporting

  • Keep track of progress-such as work hours or independent living milestones
  • Adjust plans if things aren't working and celebrate successes when they are

Training & Community Connections

  • Offer workshops for families, schools, and employers
  • Build strong relationships with local partners like vocational rehab, colleges, healthcare, housing, and employers
