As a Transition Services Supervisor, you'll support individuals with disabilities as they take exciting steps into new life phases-like moving from high school to adulthood, from family homes to more independent living, or into new vocational programs. In this role, you'll help with planning, provide guidance, and make sure each person has the support they need to reach their goals.
Bachelor's degree in human services or related field (preferred, but not required)
At least 2 years of experience in areas like transition coordination, supported living, case management, special education, or program development
Understanding of local IDD resources and transition processes
Strong organizational skills to balance multiple cases and timelines
Clear, compassionate communication-both in writing and in conversations with families, schools, and agencies
Valid Colorado driver's license and reliable transportation
