The Community Based Service Associate (CBSA) will be the first point of contact for those individuals seeking services with CKP. The CBSA will have strong knowledge of all CKP programs and services and how individuals can efficiently access those services, including the workflows for referral, intake, eligibility, screening, and evaluation. The CBSA will serve as a resource to all clients, staff, and community members in eliminating barriers for patients related to social determinants of health and the acquisition of services. The CBSA will work from the CBS Building on Ferry Street and assume responsibility for answering all incoming calls, taking messages, directing calls to appropriate staff throughout the agency, and documenting incoming referrals for follow-up later. The CBSA will monitor walk-in clients and community providers in the waiting room and alert staff when they have a visitor, and will facilitate the adequate supervision of groups and events when necessary. The CBSA will work closely with patients accessing services through the Center for Key Population (CKP) Program and will work collaboratively with the CKP Outreach Coordinators and CKP Case Managers to ensure elimination of barriers for all patients in accessing services that include primary care, specialty care, care coordination, and support services. The CBSA will assume responsibility for projects as directed by manager including specific deliverables for programs such as data entry, patient phone contact, administrative tasks, and follow up as needed.
Performs general clerical duties to include but not limited to: photocopying, faxing, sending, and receiving correspondence. Organize, coordinate and send invitations for CKP meetings and events as needed. Provide courteous customer service in person and on the phone at 33 Ferry Street, including clients, community providers, and visitors. Maintain the 33 Ferry Street Building physical operations, including facility requests, safety monitoring, equipment maintenance, supply orders, routine sanitization requests, and coordination of donations, including the donation center on Ferry Street 2nd floor. Maintain inventory of donations and supplies and provide efficient dissemination of donations and supplies to patients as needed, including mailing, delivering, or arranging delivery of items as necessary. Responsible for submitting invoices for all orders through Workday within one week Actively participate in all CKP staff meetings, program-specific meetings, and organization meetings, as well as any community or other meeting as directed by the supervisor. Establish and coordinate schedules and templates for CRVFHP (Farmers), Medication Assistant Treatment Program (MAT), Wherever You Are Program (WYA), Oasis, Ryan White Providers, CKP Events, and OASIS vans. Provide administrative support to all CKP Programs as directed by the supervisor. Participate in all projects directed by CKP team leadership, including data entry, phone calls, and follow-up. Provide support and coordination with CBOs and CHCI sites to the 4 CT Card Program by participating in meetings, conducting outreach, phone calls to participants, gift card pick up, and monitoring data. Enter HIV routine testing data for the Prevention grant in CDC SAMS efficiently and weekly. (Secure Access Management Services Portal) Organize, promote and staff outreach events to educate and raise awareness to the public regarding Center for Key Populations Programs. Working hours may include evenings and weekends as directed by the supervisor. Special assignments, projects, and other duties as assigned by the direct supervisor.
Associates degree required Experience in answering calls in a busy phone system Electronic appointment scheduling experience Strong customer service experience Excellent communication skills Basic computer expertise including data entry A valid CT driver's license; Expected to drive a company van.
