Social Services Assistant

Are you looking for an exciting, fast-paced and challenging opportunity to use your social work assessment skills as well as a service excellence focus to make a difference for our patients and caregiver Dayshift position, Monday thru Friday. Occasional evenings if needed.

Abingdon Health & Rehab Center in Abingdon, is currently searching for a dynamic Social Services Assistant to join our interdisciplinary team.

Prior human services, social services, and/or care planning experience is needed. Prior experience in a long-term care or rehab facility is a great asset.

We are looking for a Social Services Assistant to:

  • Serve as an essential member to the interdisciplinary team in developing and maintaining a patient centered plan of care addressing patient/residents identified needs and verify all disciplines are addressing planned interventions to ensure a safe transition to the next level of care.
  • Assist the Director of Social Services with the timely delivery of NOMNC & ABN Notices as well as the expedited appeals process.
  • Complete all appropriate sections of the MDS timely and accurately.
  • Assist the Director of Social Services with Quality Improvement efforts, and regulatory compliance as it relates to NOMNCs, PASRR, Advance Directives, Care Plan meetings, and Ombudsman notification.
  • Complete appropriate referrals to community agencies and resources available to meet the needs of patients throughout care transitions.
  • Demonstrate and promote a positive, collaborative, and caring service excellence team environment.

Qualifications:

  • A minimum of one (1) year supervised social work experience in a health care setting working directly with individuals is preferred
  • High School Diploma or GED
  • Strong customer service, teamwork and communication skills
  • Organized and detailed in work performance
