Virtual visits will take place out of the study's office at Anschutz medical campus, a secure location at the CHW's residence, and in-person visits will take place in participant's homes.

The CHW will: work directly (in-person and virtual) with families to help them create healthy home environments; will develop and use Motivational Interviewing skills and SMART goal setting with families; will engage in video-recorded supervision and feedback to improve their CHW skills and work with families to improve their home food environments; will oversee case management via RedCap and other platforms to advance families in the Family Matters Program; will help develop program materials for families; and other activities related to advancing families in the Family Matters program.
