Job Description
Position Summary
Provide personalized care and support to clients in the comfort of their homes, ensuring a high-quality, compassionate service that meets the individual needs of each client. Work closely with the client and their family to promote independence, safety, and overall well-being.
Key Responsibilities
- Personalized Care: Assist clients with daily activities such as bathing, dressing, grooming, and personal hygiene, while encouraging their independence.
- Nutritional Support: Plan and prepare healthy, nutritious meals tailored to the client's preferences and dietary needs. Assist with feeding if necessary.
- Household Support: Light housekeeping duties including laundry, cleaning, and organizing to maintain a safe, comfortable, and welcoming home environment.
- Companionship: Provide meaningful social interaction, engaging conversation, and emotional support to improve the client's overall quality of life.
- Health Monitoring: Observe and report any changes in the client's condition, alerting the supervisor or healthcare professionals as necessary.
- Documentation: Maintain accurate records of services provided, including dates and times, ensuring clear communication with the healthcare team.
- Specialized Care: Under the guidance of a physician or healthcare professional, assist with more specialized tasks as outlined by the Colorado Nurse Aide Practice Act.
- Other Duties: Assist with a range of tasks as needed, ensuring the well-being and comfort of the client.

Qualifications
Basic Qualifications & Experience
- High School Diploma or equivalent.
- 6 months experience working in an adult day, healthcare or home care environment that serves senior adults.
- Colorado State Certified Nursing Assistant License, in standing.
- Must possess a valid Colorado driver's license in good standing.
- Must possess current vehicle registration and proof of insurance coverage and reliable automobile.
- Must be able to read, write and speak the English language.
- Must possess excellent customer service skills.
Working Conditions/Physical Requirements
- Position requires medium-heavy physical work. Heavy lifting, pushing or pulling objects up to 100 pounds of force occasionally and/or up in excess of 45 pounds of force repeatedly to move objects.
- Stands and walks constantly during working hours with occasional sitting, kneeling, squatting, and reaching above shoulder height and fine motor control with fingers. Also requires grasping.
- Exposure to blood borne pathogens, body excretions and communicable diseases
- Is subject to frequent interruptions.
- Interacts with residents, family members and visitors frequently.
