LTSS Service Care Manager works from home and travels to conduct member assessments in Hidalgo County, TX near Edinburg, McAllen, Mission, Weslaco, Mercedes, and Harlingen, TX. Mileage reimbursement is provided.
Monday - Friday: 8 am - 5 pm (CST)
Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans/service plans for long-term care members and educates members and their families/caregivers on services and benefits available to meet member needs.
Requires a Bachelor's degree and 2 – 4 years of related experience or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position.
• 3+ years of case management experience working directly with adult populations (ages:18 - 65) • 2+ years of experience coordinating and managing medical and social services, and providing patient advocacy and education to Medicaid members • Experience in FIELD-BASED Social Worker or Case Managers role in-patient hospital, community health, outpatient mental health, or state social services settings (MHAs, LIDDA) is preferred.
Pay Range: $26.50 - $47.59 per hour
