Service Coordinator
Catholic Charities of Baltimore, Senior Communities is currently seeking a Service Coordinator, who will educate and assist older adults in accessing resources and services to support Aging in Place in an affordable apartment setting. Regularly assesses residents and determines eligibility for benefits and services. The work schedule is Monday – Friday 8:30am-4:30pm.
This position will work between three locations:
- Tuesdays and Thursday at Everall Gardens located at 6100 Everall Avenue, Overlea, MD 21206
- Mondays and Wednesdays at Kessler Park located at 4230 Hollins Ferry Road, Lansdowne, MD 21227
- Fridays at St. Mark's located at 19 Winters Lane, Catonsville, MD 21228

JOB DUTIES & RESPONSIBILITIES:
- Develops a personalized, comprehensive plan that addresses the specific needs of the individual, ensuring they have access to the necessary services to improve their overall well-being.
- Initiates a network of strong, collaborative relationships that enhances access to service, supports community integration, and improves overall outcome for the individuals served.
- Sustains increasing resident engagement, strengthened social connections, and enhanced self-advocacy, contributing to improving well-being and a more supportive environment.
- Ensures a timely, effective response that ensures the safety and well-being of residents, while coordinating the necessary support and interventions to address immediate needs and long-term care plans.
- Facilitates positive communication in resident conflicts by ensuring all parties are heard and respected.
- Proactively assists residents of the surrounding community, including caregivers, family members, colleagues, and others, by providing support as needed.
- Establishes a reliable and comprehensive record of all communications, supporting continuity of care, accountability, and effective coordination of service.
- Performs other duties as assigned.
EDUCATION & EXPERIENCE REQUIREMENTS:
- Bachelor's degree
- Experience with social services delivery, preferably with older adults, and a working knowledge of local resources.
- An equivalent combination of education and experience may be considered.

- Service-oriented, consistently resolving customer or client concerns and grievances with professionalism.
- Models excellent organizational skills and meticulous attention to detail, ensuring accuracy and efficiency.
- Demonstrates discretion and professionalism in all situations, maintaining confidentiality and integrity.
- Displays exceptional planning and organizational skills, ensuring tasks are executed efficiently.
- Possesses the ability to influence and engage staff, fostering motivation and teamwork.
- Utilizes effective conflict resolution skills to address issues and maintain a positive work environment.
- Requires proficiency in using computer systems and software to perform job function, including but not limited to, basic Windows PC, web browsing (i.e., Chrome, Internet Explorer, etc.), and Microsoft Outlook. Knowledge of other Microsoft Office applications, such as Word, Excel PowerPoint, Teams, and OneDrive is preferred.
PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
- No remote or hybrid work availability based on core and essential functions of the position.
- Adjusting or moving objects up to 50# in all directions, which may include lifting, carrying, pushing, pulling, or repositioning items as necessary to perform job duties.
- Remaining in a stationary position, which may involve standing or sitting for extended periods, as required to complete tasks or duties associated with the role.
