Community Outreach Case Worker
Sign-On Bonus: Up to $1,500
Position: Community Outreach Case Worker
Schedule: 40 hours per week, Monday–Friday (Day Shift)
No On-Call, Nights, Weekends, or Holidays required
Work Setting: You will work in the community with clients daily (not inside the agency location)
Client Support Includes:
- Linking clients to community resources such as housing and food assistance
- Meeting clients face-to-face and in their homes
- Maintaining regular in-person contact with all clients
Travel Requirements:
- Must have a personal, reliable vehicle with valid insurance
- Must have a valid Class E (MO) or Class D (IL) driver's license
- If not, you must obtain one within two weeks prior to your start date
- You will be asked to transport clients in your personal vehicle
- Mileage reimbursement provided
Caseload: May vary
Training:
- In-depth training provided during your first 90 days
- Must be able to learn quickly and have a strong ability to use technology
Preferred Experience:
- Related work with individuals suffering from mental health challenges (not required)
- You will be working with individuals who suffer from severe mental health issues
Overview
BJC Behavioral Health is a community health center that provides and coordinates behavioral health services for more than 8,000 seriously mentally ill adults and seriously emotionally disturbed children in:
- St. Louis City
- St. Louis County
- St. Francois County
- Iron County
- Washington County
As an Administrative Agent of the Missouri Department of Mental Health (DMH), BJC Behavioral Health serves as a major point of entry for people eligible for mental health services funded by DMH and acts as a gatekeeper to the public mental health system.
Preferred Qualifications
Role Purpose: Responsible for providing community support services to maximize opportunities available to people living in the community while working to recover from the serious and persistent effects of mental illness.
Responsibilities:
- Provides community support services to less complex clients using treatment plan interventions that result in positive outcomes based on individual strengths and needs
- Caseload sizes will vary
- Completes timely documentation of services that clearly describe:
- The need for the service
- The intervention provided
- Relationship to the treatment plan
- Provider of the service
- Date, time, and setting of the service
- Individual's response to the service
- Contacts individuals and/or referral sources following missed appointments to re-engage and promote recovery/resiliency efforts
- Supports individuals in crisis situations
Minimum Requirements
- Education: Bachelor's Degree – Human Services or related
- Experience: No Experience
- Supervisor Experience: No Experience
- Licenses & Certifications: Class D (IL) or Class E (MO) Driver's License
