Job Profile Summary
Location: Remote and field visits required in Harlingen, La Feria, Mercedes, Weslaco, Donna, TX SHIFT: M- F 8am-5pm, 1 hour lunch break; No OT required Duration: 6 months with possible extension
Description:
Assist in telehealth biannual assessments, in person as needed if in the area.
Complete assigned tasks and document findings.
Coordination with multi-disciplinary teams.
Follow up calls to members.
Any other projects that may arise.
Must be able to meet strict turn-around times with documentation for assessments assigned.
Candidate Requirements
Education/Certification - Required: Licensed Vocational Nurse (LVN) - Preferred:
Licensure - Required: Licensed Vocational Nurse (LVN) - Preferred:
Years of experience required: at least 1-2 years case management or home health exp Microsoft Word, Excel experience, Computer Literacy Disqualifiers: No vehicle, No internet, Performance indicators: Best VS. average: Strong Time management skills, and very well organized. Additional qualities to look for: At least 2 years experience at LVN, Home Health experience, Case management experience, speaks and understands Spanish
