Director Of Health Services - RN - Avamere Court At Keizer

The primary purpose of the Director Of Health Services is to plan, organize, develop and direct the overall operation of our Health Service Department in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be directed by the Executive Director and Regional Nurse Consultant to ensure that the highest degree of quality care is maintained at all times.

Responsibilities:

• Delegates tasks according to Oregon Board of Nursing Rules and Regulations.
• Develop and maintain a good rapport with all parties involved with the care plan.
• Report known or suspected incidents of fraud, waste, and or abuse to the Executive.
• Review the 24-hour report and health monitoring and make recommendations as needed.
• Assess the use of supportive devices with restraining qualities in accordance to established standards.
• Plan, develop, organize, implement, evaluate, and direct the health service department, as well as its programs and activities, in accordance with current Oregon Administrative Regulations and OSHA guidelines.
• Participate in the coordination of care with outside and offsite providers.
• Participate in the Continuous Quality Improvement Assist in developing improvement plans to correct identified problem areas.
• Participate in facility surveys (inspections) made by authorized government agencies as necessary or as may be directed.
• Coordinate services amongst the Interdisciplinary Team.
• Ensure that all personnel involved in providing care to the resident are aware of the resident's care.
• Implement and follow company policies and guidelines.
• Review IR reports and assist with the investigation and recommendations as needed.
• Complete Alert Log and Alert Follow guidelines for Alert Charting criteria.
• Review all physician orders (triple check system) timely.
• Complete Change in condition assessments per company policy.
• Complete Quarterly assessments per company policy.
• Complete admission assessments required by Registered Nurse per company policy.
• Follow guidelines for skin tracking, monitoring, and treatments.
• Participate in all compliance training programs.
• Participate in developing the Service Plan when residents receive health Make recommendations for service plans as needed.
• Attend and participate in care conferences as invited by Executive Director.

Requirements and Qualifications:

• Possess, as a minimum, a Nursing Degree from an accredited college or university.
• Must have, as a minimum, three (3) years of experience as a supervisor in a hospital, nursing care facility, or other related health care facility.
• Must have, as a minimum six (6) months of experience in rehabilitative and restorative nursing practices.
• Possess a current, unencumbered, active license to practice as a Registered Nurse in this state.
• Be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to nursing care facilities.
• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
• Must be able to relate to and work with the ill, disabled, elderly, emotionally upset, and at times hostile people within the facility.
• Be willing to seek out new methods and principles and be willing to incorporate them into existing nursing practices.
• Must have CPR/BLS.
