Team Lead - Assessment Services

Aspire Indiana Health is a nonprofit provider of comprehensive "whole health" services including primary medical care, behavioral health, recovery services, and programs addressing the social drivers of health such as housing and employment. Aspire has health centers in four Central Indiana counties serving Hoosiers of all ages and walks of life.

The Team Lead - Assessment Services will facilitate the collection of data for individuals who present for same day services. They will help determine the individual's needs for same day access services, identify and connect the individual to resources within the community as well as educate on all of Aspire services. This position will assist the Supervisor and Clinical Manager in the coordination of clients as part of the Same Day Access process and patient navigation follow up services. They will also assist in onboarding and ongoing training and support for the team. They will provide services via video conference, telephonically, and in person at one of Aspire's Outpatient offices and may be asked to cover different offices based on need.

Education and Experience

  • Bachelor's degree in social work, psychology, counseling or a directly-related human service field from a college/university accredited by the US Department of Education required
  • Two (2) years directly related experience in past professional and/or nonprofessional roles required
  • One (1) year supervisory experience and/or leading a team preferred
  • Experience with Crisis Intervention highly preferred
  • Knowledge of community resources, referral processes, social service systems (Medicaid, Social Security, Corrections, etc) and risk assessments preferred
  • May self-identify as a current or former recipient of services, and/or a family member of an individual who was a recipient of services, and be maintaining a healthy recovery from mental illness and/or substance use to qualify for the peer track

  • Strong ability to maintain strict confidentiality and handle highly confidential information with professionalism
  • Must possess/maintain a valid driver's license, current automobile insurance and a driving record that meets the guidelines and requirements of the organization
  • Must have access to reliable transportation to transport self/others and to attend trainings and/or meetings
