How You'll Make An Impact In This Role

• Conduct client intake, eligibility screening, and social history assessments.
• Develop and implement individualized service plans based on identified needs and goals.
• Coordinate access to appropriate social and community services.
• Monitor progress and resolve client issues through ongoing follow-up.

What Minimum Requirements You'll Need

Licensure / Certification / Registration:

Preferred Credential(s): Social Worker.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Bachelor's degree OR 4 years of applicable cumulative job specific experience required.

What Additional Preferences We're Seeking

• Social Service Designee certificate (SSD)
