With the support of the community, Hospice of the Valley's Dementia Care and Education Campus is an innovative and comprehensive resource for those living with Alzheimer's disease and other dementias. The Campus will provide full-spectrum care for persons living with all stages of dementia and support for their families. A 12-person Assisted Living Center, a 10-person Hospice Inpatient Care Home, an Adult Day Center, and a Child Center will provide superb and creative person-centered care, and a Community Education Center will educate and inspire physicians, nurses, social workers, caregivers, families, and the community to improve quality of life for persons living with dementia.
This is an exciting opportunity to be part of our state-of-the-art innovative Dementia campus. This Campus LPN, will be work in conjunction with a RN to coordinate care at our Assisted Living Facility while also providing backup nursing support for our Adult Day program. The Campus LPN will be mentoring staff in providing excellence in person-centered dementia care for persons living with dementia. This person will also participate in providing education for clinical professionals (physicians, residents, medical students, nurses, social workers, nursing assistants) and for family caregivers and community members using the facility staff and residents as models, demonstrating, mentoring, and teaching excellence in person-centered care.
