With the support of the community, Hospice of the Valley's Dementia Care and Education Campus is an innovative and comprehensive resource for those living with Alzheimer's disease and other dementias. The Campus will provide full-spectrum care for persons living with all stages of dementia and support for their families. A 12-person Assisted Living Center, a 10-person Hospice Inpatient Care Home, an Adult Day Center, and a Child Center will provide superb and creative person-centered care, and a Community Education Center will educate and inspire physicians, nurses, social workers, caregivers, families, and the community to improve quality of life for persons living with dementia.

Position Profile

This is an exciting opportunity to be part of our state-of-the-art innovative Dementia campus. This Campus LPN, will be work in conjunction with a RN to coordinate care at our Assisted Living Facility while also providing backup nursing support for our Adult Day program. The Campus LPN will be mentoring staff in providing excellence in person-centered dementia care for persons living with dementia. This person will also participate in providing education for clinical professionals (physicians, residents, medical students, nurses, social workers, nursing assistants) and for family caregivers and community members using the facility staff and residents as models, demonstrating, mentoring, and teaching excellence in person-centered care.

Responsibilities with RN Oversight

• Develops and maintains therapeutic relationships.
• Collaborates in providing care; coordinates with RN leadership and facility Physician to plan care and intervene effectively in the care of the Assisted Living residents, Adult Day members and their families.
• Creates timely and accurate documentation.
• Supports and mentors care staff on medication management and administration for the residents at the Assisted Living.
• Provides effective pain/symptom assessment and management
• Promotes patient safety.
• Facilitates smooth transition between care environments.
• Provides effective patient/family/caregiver teaching.
• Participates in providing education to clinical professionals, family, caregivers, and community members.
• Adheres to scope of practice/ HOV standards and facilitates continuously improved processes/services.
• Maintains and enhances professional skills.
• Adheres to high standards of personal and professional conduct.

Minimum Qualifications

• Minimum of two years' experience in working with persons with dementia.
• Advanced knowledge in the care of patients with dementia and delirium.
• Advanced knowledge in the optimal design and operation of facilities to meet the needs of persons living with dementia.
• Knowledge of federal and state regulations as they apply to healthcare.
• Must be able to communicate articulately and accurately, interpret and explain complex information and comprehend written and verbal communication.
• Must have highly developed interpersonal skills, ability to meet people with ease, to write and edit effectively, to communicate the written and spoken word with tact, diplomacy and/or authority when necessary.
• Ability to calculate figures and perform basic mathematical functions.
• Must demonstrate advanced critical thinking skills including the ability to interpret a variety of information furnished in multiple ways, to assess accurately and to choose a reasonable action.
• Possess professional image, enthusiasm and self-motivation.
• Ability to manage time well, meet deadlines and perform with attention to detail, speed, accuracy and follow-through.
• Excellent computer skills in a Microsoft Windows environment

Preferred Qualifications

• Arizona Licensed Practical Nurse, preferred.
• Hospice or Long-term care facility experience, highly preferred.
