Responsibilities include distribution, drug therapy monitoring, education and supervisory functions. Must collaborate with others, organize well, accomplish tasks, solve problems and communicate effectively. The individual must demonstrate knowledge of the principles of growth and development of the life span. The ability to assess data reflective of the patient's status and appropriately interpret information relative to the patient's age-specific needs is required.

Organizational Relationships: Receives direction from the pharmacy administrator. Works cooperatively with pharmacy leadership and staff, and other Mayo Clinic staff. Delegates/assigns duties within pharmacy team and precepts pharmacy experiential learners (e.g. students, interns, technician students, residents) as applicable. Serves as an education resource for staff.

Position Overview (Major Functions and Non-Essential Functions): Provides medication management services in coordination with other health care professionals to deliver safe, appropriate, and cost-effective medication therapy to patients. Responsibilities may include assessment of medication therapy regimens, medication therapy management, medication order verification/entry, medication distribution, medication therapy monitoring, and consultation, documentation of medication therapy plans, and medication use education and pharmaceutical care services.

Participates in collaborative practice and/or protocol-driven practice as appropriate. Additionally, pharmacists are active members in quality improvement, research activities, and education as described in job specific competencies.

Qualifications:

Minimum Education and/or Experience Required:

  • Graduate of an accredited College of Pharmacy with a Doctor of Pharmacy (Pharm.D.) degree OR a Bachelor of Science (B.S.) AND 2-years relevant experience.

Additional Preferred Experience and/or Qualifications:

  • Completion of Ambulatory Care PGY2 residency preferred or equivalent Ambulatory Care experience.
  • Board Certified Ambulatory Care Pharmacist (BCACP) preferred.
  • ASHP Weight Management Certificate (or an accredited Medication Therapy Management Certificate Program) preferred.
  • Evidence of ongoing scholarly pursuits including research/quality improvement, publications, or scientific presentations preferred.
  • Relevant applicable pharmacy experience working in a progressive and collaborative practice environment preferred.
  • Experience and/or interest in teaching and/or precepting preferred.
  • Commitment to excellence, accuracy, attention to detail and teamwork required.
  • Ability to communicate effectively, work collaboratively with others, organizes time well, practice with an agile mindset, solve problems, and work independently with minimal supervision required.

License and Certification:

Current Pharmacist license by the Board of Pharmacy in the state of Arizona.

Exemption Status: Exempt

Compensation Detail: Education, experience and tenure may be considered along with internal equity when job offers are extended. $144,000 - $174,000 annually
