Director Of Clinical Services, Pace Program

The Position

  1. The Director of Clinical Services, PACE Program, will lead clinical operations for approximately 205+ PACE participants, overseeing primary care, risk management, quality, and safety operations for HopeWest.
  2. Reporting directly to the Chief Operating Officer, this leader will oversee approximately 50+ FTEs including RNs, CNAs, and in close collaboration with physician and IDT teams.
  3. In collaboration with fellow Director's, the Director will focus on ongoing clinical initiatives including care plans, behavioral health initiatives, and case reviews.
  4. Key responsibilities for this leader include fostering trust through strong staff relationships, assessing team dynamics, identifying and removing barriers, leveraging data to drive decisions, and providing recommendations for improvement and positive change. Additional responsibilities include clinical oversight related to workflow development, fall risks, pharmacy workflows, committee reviews, and finances/invoices.
  5. The ideal candidate is a hands-on, collaborative leader with a proven ability to build strong relationships and drive results. Additionally, this leader must be adaptive, communicative, and bring exceptional decision-making and business acumen skills to ensure that the program can achieve both long- and short-term goals.

Requirements

  • BSN is required; MSN is preferred.
  • Active CO and/or compact RN license and/or ability to require is required.
  • A minimum of five years of clinical nursing experience including a minimum of three years of leadership experience is required.
  • Critical Care experience is strongly preferred.
