We are seeking a dedicated and experienced Registered Nurse (RN) to join our leadership team as an Assistant Director of Patient Services. This role is ideal for a compassionate and detail-oriented RN with a strong history in home care and supervisory responsibilities. The successful candidate will play a key role in ensuring high-quality patient care, regulatory compliance, and operational excellence.

Qualifications:

• Current RN license in good standing (New York State preferred)
• Minimum 3 years of nursing experience, preferably in home care or community health
• Proven supervisory or leadership experience
• Proficiency in UAS-NY assessments and home care evaluations
• Strong organizational and communication skills
• Experience with chart audits and regulatory preparedness is a plus

Key Responsibilities:

• Conduct initial assessments for new home care cases
• Oversee and review documentation submitted by field nurses to ensure accuracy and compliance
• Prepare for audits by maintaining up-to-date and compliant patient records
• Perform chart audits and implement quality improvement measures
• Collaborate with the Director of Patient Services to support staff development and operational goals
• Serve as a clinical resource and mentor to nursing staff

Pay and Benefits

The pay range for this position is $140000.00/yr. Paid time off, Holiday Pay, Union
